Job summary

Harmony Senior Services in Bridgeport, WV, seeks a licensed practical nurse to provide quality nursing care for residents in an assisted living setting. The LPN coordinates physical, social, and emotional aspects of care per physician orders and resident choices.

Responsibilities include medication management, ongoing assessments, safety documentation, and collaboration with the nursing team. Must lift up to 50 lbs and be willing to work flexible shifts, including weekends.

Qualifications

• Current LPN license to practice nursing in the state where the community is located.
• Experience working with the senior population, preferably in assisted living or LTC.
• Must be a team player who works well with others and has experience supervising staff.
• Must possess critical thinking skills, ability to effectively communicate verbally and in writing.
• Demonstrated ability to manage time efficiently with strong organizational skills.
• Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs.
• Must be willing to work an 8-16 hour flexible schedule to include weekends and holidays.
• Must attend all required trainings, CEU's and staff meetings and be certified or willing to take state-required certification.

Responsibilities

• Execute the medication management plan with documentation and communication to the physician as needed.
• Continuously assess resident needs and update care, services and condition changes.
• Implement measures to ensure resident wellness and safety with proper documentation.
• Work in a team to meet resident needs and deliver care.
• Respond to emergencies promptly and calmly, with appropriate reporting.
