Job summary

Harmony Senior Services in Bridgeport, WV seeks a qualified LPN to provide quality nursing care to residents in assisted living. You will coordinate physical, social, and emotional care under physician orders, ensuring safety and dignity.

The role requires an active team approach, good communication, and the ability to respond to emergencies. Flexible 8-16 hour shifts including weekends/holidays, with training and career development opportunities.

Qualifications

• Current LPN license to practice in the state where employed (West Virginia).
• Experience working with the senior population in assisted living or LTC.
• Team player with experience supervising staff.
• Strong verbal and written communication skills.
• Good time management and organizational abilities.
• Able to sit, stand, bend, kneel, walk and lift up to 50 lbs.
• Willingness to work an 8-16 hour flexible schedule including weekends and holidays.
• Willingness to attend required trainings and CEUs and pass state certification classes.

Responsibilities

• Execute the medication management plan with documentation review and physician communication as needed.
• Continuously assess resident needs and update caregivers accordingly.
• Maintain resident safety and accurate documentation of measures taken.
• Collaborate with the healthcare team to meet resident needs.
• Respond to emergencies in a timely, calm, and competent manner.
• Report incidents and updates to the Healthcare Director with proper documentation.
• Participate in activities and community events with residents.
• Assist with meals, monitor diet acceptance, and ensure adequate nutrition.
• Maintain high standards of record keeping for residents and staff.
• Establish positive relationships with all departments and identify risk factors.
• Provide thorough shift-change reports and daily logs.
• Motivate and supervise CNAs/RMAs and support ongoing training.
