UnitedHealth Group is seeking a Licensed Practical Nurse for Home Health to provide quality patient care in the patient’s home and perform skilled nursing tasks as defined by the plan of care. The role emphasizes infection control, documentation, and coordination with the care team.

Primary responsibilities include delivering high-quality clinical services, thorough charting, and active participation in care coordination while maintaining flexibility to meet patient needs.

Qualificações

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilidades

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, reporting patient needs and changes in condition, and communicating with patient, family, physician and team.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
