Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• Home care experience.

Responsibilities

• Provides high quality clinical services within the scope of practice in accordance with the plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Acts in care coordination, reporting needs and changes in condition to the team.
• Works with patients from diverse backgrounds and in various home settings.
