RVNAhealth in Connecticut is seeking a Licensed Practical Nurse to provide direct patient care in clients' homes across Central Fairfield County, including Wilton, Weston, Redding and Ridgefield. Qualifications

• Active LPN license in Connecticut required.
• Minimum 2 years clinical nursing experience, home care preferred.
• Strong knowledge of medical terminology and Medicare/insurance documentation.
• Proven communication and multitasking skills.
• Proficient with electronic health records and documentation.

Responsabilités

• Deliver direct patient care in clients' homes as assigned.
• Maintain updated care plans and report to RN Case Manager and physician.
• Educate patients and families about diagnoses, medications, and treatments.
• Use and maintain medical equipment and nursing supplies.
• Document patient information using EMR/EHR systems.
• Participate in on-call rotation and support clinical staff as needed.
• Escalate emerging issues to leadership and ensure quality of care.
• Build trusting relationships with patients and families.
