Job summary

RVNAhealth is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes across Southern Fairfield County, including Norwalk, Stamford and Greenwich. Under RN supervision, you will implement care plans and advocate for clients.

The role requires CT LPN license, graduation from an accredited program, at least 2 years of clinical nursing experience (home care preferred), and solid documentation in electronic health records.

Qualifications

• Graduate of an accredited nursing program.
• Active LPN license in the State of Connecticut.
• Minimum of 2 years clinical nursing experience (home care experience strongly preferred).
• Strong understanding of medical terminology and Medicare/insurance documentation requirements.
• Calm under pressure with strong organizational and multitasking skills.
• Excellent interpersonal communication and customer service skills.
• Proficient with electronic health records and computer documentation.
• Must have reliable transportation and the ability to travel in all weather conditions.

Responsibilities

• Deliver skilled, direct patient care as assigned, while communicating timely updates to the RN Case Manager, physician, and Clinical Supervisor.
• Monitor and update nursing care plans based on physician directives.
• Educate clients and families about diagnoses, medications, and treatment plans.
• Use and maintain medical equipment and nursing supplies properly and efficiently.
• Document patient information accurately using electronic medical records.
• Participate in the on-call rotation and provide support to other clinical staff as needed.
• Identify and elevate any emerging clinical issues to the appropriate leadership.
• Build strong, trust-based relationships with patients, families, and team members.
• Support agency quality improvement efforts and participate in ongoing professional development.
