Job summary

RVNAhealth is seeking an Licensed Practical Nurse (LPN) to provide hands-on nursing care in patients' homes across Southern Fairfield County, including Norwalk, Stamford, and Greenwich. Under RN supervision, you will advocate for patients and ensure care plans are followed and documented.

The role emphasizes compassionate care, documentation accuracy, and collaboration with the RN Case Manager, physician, and Clinical Supervisor to maintain high-quality outcomes.

Qualifications

• Graduate of an accredited nursing program.
• Active LPN license in the State of Connecticut.
• Minimum of 2 years clinical nursing experience (home care preferred).
• Strong understanding of medical terminology and Medicare/insurance documentation requirements.
• Calm under pressure with strong organizational and multitasking skills.
• Excellent interpersonal communication and customer service skills.
• Proficient with electronic health records and documentation.

Responsibilities

• Deliver skilled, direct patient care in patients' homes as assigned.
• Update care plans based on physician directives and RN input.
• Educate clients and families about diagnoses, medications and treatment plans.
• Document patient information accurately in EMR systems.
• Participate in on-call rotation and support other clinical staff.
