Job summary

RVNAhealth in Connecticut is seeking a Licensed Practical Nurse (LPN) to provide hands-on home health care under RN supervision in Wilton, Weston, Redding, Ridgefield and surrounding towns. You will assess patients, implement plans, educate families, and document care using EMR.

The role requires a graduate of an accredited nursing program and CT LPN license, with 2 years of clinical nursing experience preferred.

Qualifications

• Graduate of an accredited nursing program.
• Active LPN license in the State of Connecticut.
• Minimum of 2 years clinical nursing experience (home care preferred).
• Proficient with electronic health records and documentation requirements.
• Travel required to multiple towns within service area.

Responsibilities

• Deliver skilled, direct patient care in home settings as assigned.
• Monitor and update care plans based on physician directives.
• Educate clients and families about diagnoses, medications, and treatment plans.
• Document patient information accurately using EMR.
• Participate in on-call rotation and support other staff.
• Communicate updates to RN Case Manager, physicians, and supervisor.
• Identify and escalate emerging clinical issues to leadership.
