Constellation Health Services in Stamford/Greenwich, CT is seeking a dedicated Licensed Practical Nurse (LPN) to join our home care team. You will follow the plan of care under RN supervision, provide treatments, and educate patients and families in their home and community settings.

The ideal candidate has an accredited practical nurse program graduate, an active LPN license in CT, and 1–2 years of home care experience.

Qualifications

• Graduate of an accredited practical nurse program.
• Licensed LPN in the state of employment (CT).
• 1–2 years of LPN home care experience preferred.
• Ability to manage catheters, ostomy care, and related tasks.
• Reliable transportation for in-home visits.

Responsabilités

• Provide nursing care in home or community settings as per plan of care.
• Follow plan of care established by physician under RN supervision.
• Educate patient and family about diagnosis and care plan.
• Document services accurately and timely.
• Coordinate with hospice IDT and other care team members.
