Job summary

UnitedHealth Group is seeking a Full-Time Home Health LPN who travels throughout Stamford, CT and surrounding towns while managing a local caseload.

The Licensed Practical Nurse will provide quality home health nursing, document visits, and coordinate care with the health team in accordance with regulatory requirements.

Required: current LPN license in CT and valid driver's license; CPR certification preferred or to be obtained within 90 days of hire. Home care experience is a plus.

Qualifications

• Active LPN license in CT required.
• Current driver's license with access to reliable transportation.
• Strong communication and organizational skills.
• Ability to work independently in patients' homes.
• CPR certification preferred or within 90 days.

Responsibilities

• Provide high-quality clinical nursing services in patients' homes per plan of care and infection control procedures.
• Document visits thoroughly in the EMR and verify regulatory/payer requirements.
• Coordinate care with the health team and communicate with patient, family, and providers.
• Function effectively in diverse home settings and populations.
