Evergreen Center for Health & Rehabilitation is hiring aLicensed Practical Nurse (LPN) to provide direct nursing care to residents, administer medications, and collaborate with the care team to execute individualized care plans.

Qualifications

• Active LPN license in good standing
• Demonstrated compassionate patient care in long-term or skilled nursing settings
• Excellent communication with residents, families, and care teams
• Ability to work effectively in a multidisciplinary team

Responsibilities

• Administer medications and treatments as prescribed
• Monitor and record patient vital signs and health information
• Collaborate with interdisciplinary team to implement care plans
• Provide wound care and other nursing interventions as needed
• Supervise and support CNAs in delivering care
• Ensure compliance with regulatory standards and protocols
