Job summary

Independence Rehab and Nursing is seeking a compassionate Licensed Practical Nurse (LPN) to join our dedicated team on site. You will provide hands-on resident care, administer medications, and collaborate with an interdisciplinary team to support residents’ strength and dignity.

Qualifications

• Licensed Practical Nurse (LPN) required
• Experience in resident care is preferred

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and detailed records of patient care and progress
• Participate in quality improvement initiatives and peer review processes
• Collaborate with other healthcare professionals to ensure comprehensive care and achieve desired outcomes
• Provide support to colleagues and contribute to a positive and respectful work environment
