Job summary

Evergreen Center for Health & Rehabilitation in Stafford Springs, CT, is seeking a Licensed Practical Nurse (LPN) to join a compassionate, professional team. You will provide direct nursing care, administer medications, monitor residents, and help implement individualized care plans to promote comfort and recovery.

As part of National Health Care Associates, you’ll work with CNAs and interdisciplinary clinicians in a supportive, collaborative environment that values kindness and excellence.

Qualifications

• Valid state LPN license.
• Compassionate and empathetic approach to patient care.
• Interest in aging and chronically ill patients; ability to work with diverse teams.
• Strong clinical nursing skills including medication administration.
• Excellent communication and interpersonal abilities.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and record vital signs and medical information.
• Collaborate with interdisciplinary team to implement care plans.
• Provide wound care and other nursing interventions.
• Supervise and support CNAs in delivering care.
• Ensure compliance with regulatory standards and protocols.
