Job summary

Harmony Senior Services, LLC seeks a dedicated LPN to provide compassionate nursing care in Avon, Indiana. You will coordinate medical services, monitor resident wellness, and support dietary and activity programs within a collaborative team.

Requirements include an active LPN license, experience with seniors, and the ability to work a flexible schedule with weekend/holiday coverage. Training and ongoing CEUs are expected.

Qualifications

• Must hold a current LPN license in the applicable state.
• Experience with senior care, assisted living, or LTC preferred.
• Strong verbal and written communication skills.
• Demonstrated time-management and organizational abilities.
• Ability to lift up to 50 lbs and perform physical tasks as needed.

Responsibilities

• Provide quality nursing care and coordinate medical services for residents.
• Execute medication management plan and communicate changes to physicians.
• Assess residents and update care plans with team input.
• Maintain safety, documentation, and regulatory compliance.
• Collaborate with CNA/RMA staff and participate in meals and activities.
• Report incidents and changes in condition to Healthcare Director and log daily notes.
