Job summary

Blue Central Group is seeking a dedicated Licensed Practical Nurse (LPN) to join our long-term care team. You will provide high-quality nursing care to residents in a compassionate environment and ensure compliance with long-term care regulations.

The role includes administering medications, completing assessments, and collaborating with interdisciplinary teams to create individualized care plans. Prior geriatric experience and proficiency with PCC EHR are highly valued.

Qualifications

• Must hold a valid LPN license in the state of practice.
• Experience in long-term care facilities such as nursing homes or SNFs is preferred.
• Proficient in medical terminology, anatomy, and physiology; ICD coding knowledge a plus.
• Experience with EMR/EHR systems (e.g., PCC) for accurate documentation.

Responsibilities

• Administer medications, following physician orders and safety protocols.
• Conduct comprehensive patient assessments, monitor vitals, and document medical records using EHR.
• Assist with activities of daily living including tube feeding, catheter care, and mobility with Hoyer lifts.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Manage acute pain and coordinate hospice or palliative care services as needed.
• Ensure infection control policies are followed, including specimen collection, sterilization, and aseptic techniques.
• Communicate with residents, families, and team members about health status and care plans.
