Job summary

Qualifications

• Licensed practical nurse license required (LPN/LVN).
• Experience in diverse settings (hospitals, nursing homes, hospice, clinics).
• Familiarity with PCC EMR and ICD-9/10 coding advantageous.

Responsibilities

• Conduct comprehensive patient assessments and document vital signs and health history.
• Administer medications per protocols and safety standards.
• Perform procedures such as catheterization, wound care, and sterile technique as needed.
• Monitor patient conditions and triage when deterioration occurs.
• Collaborate with care team to implement individualized care plans for geriatrics and dementia care.
