Lutheran Senior Services is seeking an experienced Licensed Practical Nurse (LPN) to provide and coordinate comprehensive nursing care for residents, under the state Nurse Practice Act and EverTrue policies. You will work with an interdisciplinary team to develop individualized care plans, administer medications, monitor IV therapy, and document care in the EMR, ensuring safety and quality.

Qualifications

• Licensed Practical Nurse license in the state of practice.
• CPR certification required.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practice nursing within licensure and act according to Nurse Practice Act; keep license active.
• Collaborate with the interdisciplinary team to define individualized care plans.
• Pass or supervise medication administration and monitor IV fluids and treatments.
• Initiate IV therapy if IV-certified.
• Maintain observation, assessment, and documentation including care plans and MDS.
• Use EMR for complete clinical documentation.
• Monitor resident status and coordinate with services for continuity of care.
• Maintain a safe, functional environment and address hazards.
• Oversee certified staff to meet EverTrue standards.
• Report incidents per state and federal regulations.
