Lutheran Senior Services is seeking a Licensed Practical Nurse (LPN) to provide and coordinate comprehensive care for residents in a skilled nursing setting. The role emphasizes nursing within licensure scope, collaboration with an interdisciplinary team, and ensuring high-quality clinical care.

The LPN will administer medications, monitor IV therapy where certified, document thoroughly in the EMR, and contribute to care planning and regulatory compliance.

Qualifications

• LPN licensure in state of practice is required.
• CPR certification is required.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practices nursing care within the scope of licensure and state Nurse Practice Act.
• Coordinates individualized care plans with the interdisciplinary team.
• Passes medications and oversees administration and IV therapy where applicable.
• Maintains accurate documentation in the electronic medical record.
• Ensures a safe, functional environment and coordinates with other services.
