Qualifications

• Current LPN license required.

Responsibilities

• Develop and implement individualized care plans with the interdisciplinary team.
• Communicate with residents, families, and healthcare professionals to ensure comprehensive care.
• Monitor and document patient progress and changes as needed.
• Participate in ongoing education to stay current with practices and regulations.
