WeCare Centers is hiring a Licensed Practical Nurse (LPN) to deliver high-quality nursing care under RN supervision in a supportive setting. You will administer medications, monitor residents, and document care while assisting with admissions and assessments.

The role emphasizes collaboration with physicians and CNAs to create individualized care plans, ensuring compliance with regulations, and fostering a compassionate environment for residents and families.

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing.
• Long-term care or skilled nursing experience preferred but not required.
• Strong clinical assessment, critical thinking, and organizational skills.
• Excellent communication and teamwork abilities.
• CPR certification (or ability to obtain).

Responsibilities

• Deliver high-quality nursing care under the direction of the RN and attending physician.
• Administer medications and treatments safely and accurately.
• Monitor residents' conditions and report changes to the healthcare provider.
• Complete accurate and timely clinical documentation.
• Assist with admissions, discharges, and resident assessments.
• Collaborate with physicians, nurses, therapists, and other team members to develop care plans.
• Supervise and support CNAs while promoting teamwork and resident care.
• Ensure compliance with state and federal regulations and facility policies.
• Foster a safe, respectful, and compassionate environment for residents and families.
