Job summary

Mercy Health is seeking a Licensed Practical Nurse (LPN) to deliver direct patient care under supervision of an RN. The role focuses on patient assessments, care coordination, education, and clear communication with the healthcare team to ensure quality, compassionate care.

Key responsibilities include admissions/discharges, ongoing care, accurate EMR documentation, and participation in unit meetings and safety initiatives.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current LPN license in the state of practice.
• Ability to meet all State Board of Nursing requirements.

Responsibilities

• Assist with patient admissions, discharges, and ongoing care.
• Perform patient assessments and monitor changes in condition.
• Communicate patient updates and concerns to the RN and care team.
• Participate in bedside handoffs and patient safety initiatives.
• Provide patient and family education and support.
• Document care accurately in the electronic medical record.
• Collaborate with the interdisciplinary team to deliver high-quality care.
• Delegate and direct support staff activities as appropriate.
• Participate in unit meetings, huddles, and process improvement activities.
• Perform additional duties as assigned.
