Job summary

Mercy is hiring a Licensed Practical Nurse (LPN) to provide direct patient care under the supervision of an RN. The role supports patient assessments, care coordination, education, and effective communication within the healthcare team.

Responsibilities include admissions and discharges, ongoing care, documenting in the EMR, and collaborating with the interdisciplinary team to ensure high-quality care. A valid LPN license and state nursing requirements are required; BLS and other certifications

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current LPN license in the state of practice.
• Ability to meet all State Board of Nursing requirements.

Responsibilities

• Assist with patient admissions, discharges, and ongoing care.
• Perform patient assessments and monitor changes in condition.
• Communicate patient updates and concerns to the RN and care team.
• Participate in bedside handoffs and patient safety initiatives.
• Provide patient and family education and support.
• Document care accurately in the electronic medical record.
• Collaborate with the interdisciplinary team to deliver high-quality care.
• Delegate and direct support staff activities as appropriate.
• Participate in unit meetings, huddles, and process improvement activities.
• Perform additional duties as assigned.
