Job summary

Amedisys is seeking a Licensed Nursing Assistant to serve as a LNA Hospice Aide in the Concord, NH area. You will provide compassionate personal care, assist with daily activities, and support patients and families in their homes.

Responsibilities include observing patient status, following RN plans of care, assisting with meals and exercises, and maintaining accurate documentation. Strong CNA credentials and CPR certification are required, with a valid driver’s license.

Qualifications

• Must provide personal care services and observe patient status.
• Follow plan of care written by RN and report changes.
• Maintain safety, proper body mechanics, and documentation.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and body mechanics during care and transfers.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental services as assigned.
• Observes, reports, and documents changes in patient status.
• Follow RN instructions for personal care per plan of care.
• Assists with prescribed exercises and basic tests as directed.
• Performs and records simple urine tests as required.
• Other duties as assigned.
