Job summary

Home Health & Hospice Care in Merrimack, NH, seeks a Licensed Nursing Assistant for the Community Hospice House on a Monday–Friday 3:00pm–11:00pm shift with weekend/holiday rotation. You will work under an RN to support patient comfort and safety in a compassionate, patient-centered team.

Responsibilities include personal care, safe mobility, careful documentation, and clear communication with patients and families.

Qualifications

• Licensed Nursing Assistant in NH in good standing.
• CPR certification is required.
• Reliable, compassionate and patient-centered care approach.
• Preferred: home care experience and dementia/COPD/cancer/post-surgical exposure.

Responsibilities

• Provide personal care (bathing, dressing, grooming, toileting) and safe transfers.
• Assist mobility using mechanical aids and proper body mechanics.
• Observe and report changes in condition and patient status.
• Cue patients for medications as directed in care plan.
• Maintain clean, safe environment following aseptic standards.
• Communicate professionally with patients, families, and care team.
• Complete timely and accurate documentation.
• Uphold professional boundaries and customer service.
