Job summary

Brookhaven Hospice in New Hampshire is seeking a full-time Hospice Aide (LNA) to join our compassionate team. You will provide hands-on, patient-centered care under the nursing team's supervision, helping with daily living activities and comfort.

The role covers Concord, Hooksett and Manchester, with a Monday–Friday, 8 am–4:30 pm schedule. We offer comprehensive benefits and growth opportunities within our mission-driven organization.

Qualifications

• High School Diploma or GED required.
• Unencumbered LNA license in the state.
• Minimum 1 year experience as an LNA in an acute care or LTC setting; hospice experience a plus.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Provide personalized care following the plan of care to ensure patient comfort and well-being.
• Assist with personal care, including bathing, hygiene, and grooming.
• Be dedicated to exceptional customer service and patient satisfaction.
• Support families by sharing knowledge and guidance for caring for terminally ill loved ones.
• Maintain accurate documentation in patient home and agency records.
