Job summary

Granite VNA, Inc. in Concord, NH is seeking a compassionate Licensed Nursing Assistant (LNA) to join our Hospice House team. You’ll provide comfort, dignity, and quality-of-life care to residents while supporting families and collaborating with our clinical team.

The role involves daily activities, observation, documenting changes, and promoting a safe, welcoming environment. Employment is full-time with weekend overnight schedule Friday-Sunday, 7pm-7am and a comprehensive benefits package.

Qualifications

• High School Diploma/GED with current New Hampshire LNA license.
• One year of recent experience as a Licensed Nursing Assistant.

Responsibilities

• Provide compassionate assistance with daily activities (bathing, grooming, eating, mobility, transfers, incontinence care).
• Follow individualized care plans and observe, document, and report changes promptly.
• Create a safe, clean, comfortable, and welcoming environment.
• Support residents with cognitive needs and encourage participation in meaningful activities.
• Communicate professionally with residents, families, nurses, and care team members.
• Work independently while contributing to a supportive team environment.
• Follow safe work practices, agency policies, and LNA scope of practice.
