Job summary

Leoforce is seeking a Hospice Licensed Nurse Assistant in Merrimack, NH for 2nd shift. The LNA will assist patients with activities of daily living, provide comfort measures, and work with RNs and hospice team to deliver dignified end-of-life care.

Candidates must be licensed as a NH LNA, CPR certified, pass background check and ensure safety. Home-health setting, independent work with a supportive clinical team; prior home care or dementia care preferred.

What You Need to Apply

• A Licensed Nursing Assistant in good standing with the New Hampshire Board of Nursing.
• CPR certified, with a satisfactory background check and motor vehicle record.
• Reliable, compassionate, and committed to delivering exceptional patient‑centered care.
• Comfortable working independently in the home setting while collaborating with a supportive clinical team.

Preferred: experience in home care and/or caring for patients with dementia, Alzheimer’s disease, COPD, cancer, or post‑surgical needs; ability to perform PT/INR checks.

A bit about us:

Provide compassionate, hands‑on care to patients and families in a community hospice house setting. The LNA will assist patients with activities of daily living, personal care, comfort measures, mOur Clientlity, and maintaining a safe and supportive environment. This role works closely with RNs and the interdisciplinary hospice team to ensure patients receive dignified, individualized end‑of‑life care.
