Job summary

Leoforce is seeking a Hospice Licensed Nurse Assistant in Merrimack, NH for 2nd shift. The LNA will assist patients with activities of daily living, provide comfort measures, and work with RNs and hospice team to deliver dignified end-of-life care.

Candidates must be licensed as a NH LNA, CPR certified, pass background check and ensure safety. Home-health setting, independent work with a supportive clinical team; prior home care or dementia care preferred.

Qualifications

• Licensed Nursing Assistant in good standing with NH Board
• CPR certified with background check and motor vehicle record
• Reliable, compassionate, and committed to patient-centered care
• Comfortable working independently in home setting with clinical team
