Job summary

Home Health & Hospice Care seeks a Registered Nurse to join the Community Hospice House in Merrimack, NH. You will perform skilled nursing care, comprehensive assessments, and collaborate with physicians, families, and the interdisciplinary team to ensure high-quality, goal-directed care that emphasizes comfort and safety.

The role requires an Associate Degree in Nursing (BSN preferred), NH RN licensure, and at least one year of acute care experience.

Qualifications

• Associates or better in Nursing required.
• Bachelor's degree in Nursing preferred.
• 1 year of acute care experience preferred.
• Registered Nurse license in good standing.

Responsibilities

• Deliver skilled nursing care and comprehensive assessments.
• Develop individualized care plans with measurable goals.
• Coordinate interdisciplinary care and communicate with physicians.
• Teach patients and families on self-care and safety.
• Document timely, accurate clinical notes.
• Support quality outcomes and appropriate visit utilization.
• Participate in staff meetings and quality improvement activities.
• Precept students when requested.
