Job summary

The Licensed Nursing Assistant at Home Health & Hospice Care will work 7:00am to 3:00pm, 32 hours per week, including every other weekend and holiday rotation at Community Hospice House in Merrimack, NH. You will provide hands-on personal care under an RN, delivering compassionate end-of-life support in a recently expanded facility.

Qualifications

• Licensed Nursing Assistant in NH in good standing
• CPR certified, with a satisfactory background check
• Reliable, compassionate, and committed to patient-centered care
• Preferred: experience in home care or caring for dementia, COPD, cancer, or post-surgical needs

• Use mechanical devices and proper body mechanics to ensure safe mobility assistance.
• Observe and report changes in physical or mental condition, intake/output, and overall patient status.
• Cue patients for medications as directed in the care plan.
• Maintain a clean, safe, and healthy environment following aseptic and safety standards.
• Communicate professionally with patients, families, and the care team.
• Complete timely and accurate documentation.
• Uphold professional boundaries and represent the agency with excellent customer service.

About the Job:
The Community Hospice House recently completed a transformative expansion and renovation project to meet the growing need for compassionate, end‑of‑life care in southern New Hampshire. The facility added 6 new private patient suites, increasing its total room count from 10 to 16. As a Licensed Nursing Assistant in the Community Hospice House, you’ll play a key role in delivering compassionate, high‑quality personal car. You’ll work under the direction of an RN and collaborate closely with the care team to support patient comfort, safety, and well‑being. This role is hands‑on, relationship‑driven, and essential to helping patients remain safe and comfortable.

What You’ll Do:

Who You Are:

• A Licensed Nursing Assistant in good standing with the New Hampshire Board of Nursing.
• CPR certified, with a satisfactory background check and motor vehicle record.
• Reliable, compassionate, and committed to delivering exceptional patient‑centered care.
• Preferred: experience in home care and/or caring for patients with dementia, Alzheimer’s disease, COPD, cancer, or post‑surgical needs; ability to perform PT/INR checks.
