Job summary

LifeCare Home Health Family is seeking a Hospice Aide to provide personal healthcare and related services to patients in their homes. You will work under RN supervision, contribute to a safe and clean home environment, and cooperate with patients and families while reporting observations to the Hospice Administrator.

Responsibilities include direct patient care, safe transfers, reporting changes, infection control, and maintaining confidentiality.

Qualifications

• High School Diploma preferred; complete a state-approved nursing aide training or competency evaluation program.
• Experience: at least one year as Hospice Aide or Nursing Assistant preferred.
• Must be able to read, write and verbally report clinical information to patients, care givers and staff.

Responsibilities

• Provide direct patient care under RN supervision according to the Aide Plan of Care.
• Assist with safe transfers and ambulation per Hospice policy.
• Report changes and document care rendered to ensure continuity of care.
• Maintain infection control, and provide a clean, safe environment.
• Communicate respectfully with patients, families and staff.
• Respect patient privacy and property while delivering care.
• Assist with nutrition and fluid intake.
• Support Hospice philosophy and administrative policies to ensure quality care.
• Participate in on-call/after-hours coverage as assigned.
• Submit required paperwork in a timely manner.
• Meet productivity standards.
• Provide reliable transportation and maintain valid auto liability insurance.
