Job summary

Lifecare-Home-Health-Family-1 is seeking a compassionate Hospice Aide to provide in-home patient care under RN supervision. You will assist patients in their residence, keep the environment clean, and report observations to the Hospice Administrator and care team.

Responsibilities include safe transfers, infection control, nutrition support, and timely documentation. High School Diploma preferred or approved nursing aide training; reliable transportation required.

Qualifications

• Education: High School Diploma preferred with approved training as specified by Medicare regulations.
• Experience: 1+ year preferred in hospice, nursing home, or home health settings.
• Skills: Read, write, report clinical information clearly to care team and family.

Responsibilities

• Provides direct patient care under RN direction and according to care plans.
• Assists with safe transfers and ambulation per hospice policy.
• Reports changes and documents care to ensure continuity of care.
• Maintains infection control and creates a clean, safe home environment.
• Communicates respectfully with patients, families, and staff.
• Assists with nutrition and fluid intake and adheres to privacy policies.
• Participates in on-call/after-hours coverage as assigned; completes paperwork timely.
• Ensures reliable transportation and maintains required auto insurance.
