Job summary
VitalCaring is seeking a dedicated Hospice Aide/CNA to provide comfort, dignity, and compassionate care in patients' homes. You will support daily activities, monitor patient condition in collaboration with RNs, and document care with empathy and professionalism.
The role emphasizes presence, respect for patient preferences, and teamwork with RNs, social workers, and chaplains in a home-based setting.
Qualifications
- Completion of state-required Hospice Aide/CNA training and CPR certification.
- Ability to follow care plans, document clearly, and communicate effectively.
- Comfort working with end-of-life patients and families with empathy and professionalism.
Responsibilities
- Assist patients with daily living activities including bathing, dressing, grooming, toileting, and feeding.
- Support comfort through repositioning, mobility assistance, and skin integrity care.
- Provide light housekeeping to maintain a clean, safe, and comfortable environment.
- Offer respite support to caregivers and families.
- Observe and report changes in patient condition to the RN/Case Manager.
- Communicate with compassion and maintain confidentiality.
