Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification.
• Covid vaccination per facility policy.
• High school diploma or GED.
• Ability to read and follow written instructions.
• Understands hospice philosophy and provides terminally ill patient care.
• One year home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Provide personal care including bathing, oral hygiene, and grooming.
• Assist in dressing, feeding, and transferring as assigned.
• Observe and report patient condition to RN Case Manager.
• Support patient safety and proper use of equipment.
• Assist with mobility, range of motion, and exercises as directed.
• Help with light housekeeping and laundry duties.
