Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License and access to dependable transportation

Responsibilities

• Assists the patient, family, physician and care team to understand social factors related to health status
• Contributes to a comprehensive, integrated Plan of Care for patients daily
• Instructs team members on community resources available to assist patients
