Qualifications

• MS degree in social work from a CSWE-accredited program.
• Current state license as a social worker.
• 2+ years of clinical work experience in healthcare or hospice.
• Reliable transportation and ability to lift at least 25 lbs.

Responsibilities

• Perform psychosocial evaluations for patients and families.
• Provide ongoing counseling to patients and families during the end-of-life journey.
• Offer emotional support in alignment with the plan of care.
