Job summary

Elite Hospice, part of the Optum/LHC Group family, is seeking a part-time Social Worker to provide medical social services under physician direction. You will help patients and families understand social factors affecting health and contribute to an integrated care plan daily.

The role requires an MSW, active state social work licensure, and a valid driver's license with reliable transportation; CPR is preferred or obtainable within 90 days. Competitive benefits available.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Current driver's license with vehicle insurance and access to dependable transportation.
• Licensed Social Worker in residence state; CPR certification preferred or within 90 days of hire.

Responsibilities

• Assist patient, family and care team to understand significant social factors related to health.
• Contribute to daily development of an integrated Plan of Care.
• Instruct health care team on available community resources as needed.
