Responsibilities

• Conduct Comprehensive Assessments: Evaluate social, emotional, and environmental needs to develop personalized care plans.
• Provide Counseling and Support: Offer emotional support to patients and families addressing end-of-life issues.
• Collaborate with Interdisciplinary Teams: Work with physicians, nurses, and spiritual care providers for holistic care.
• Advocate for Patients: Ensure patient needs and preferences are respected and act as liaison.
• Coordinate Community Resources: Connect patients and families with community resources.
• Maintain Accurate Documentation: Keep detailed records in accordance with regulatory standards.
