Job summary

LifeSource Hospice is seeking a part-time Hospice Social Worker to provide psychosocial support and counseling to patients and families. The role involves psychosocial assessments, participation in IDG, care planning, and connecting families with community resources.

The position requires an LCSW in New Jersey (preferred for regulatory alignment), MSW, and at least one year in healthcare, with ability to travel to patient homes and facilities. Flexible hours and per diem coverage when needed.

Qualifications

• Licensed Clinical Social Worker (LCSW) in the State of New Jersey (preferred to meet hospice regulatory requirements)
• Master’s Degree in Social Work (MSW) from a CSWE-accredited school
• At least one year of experience in healthcare, preferably hospice, palliative care, or home health
• Knowledge of hospice philosophy and end-of-life care
• Valid driver’s license and reliable transportation
• Ability to travel to patient homes, facilities, and other care settings

Responsibilities

• Conduct initial and ongoing psychosocial assessments of hospice patients and families
• Participate in the Interdisciplinary Group (IDG) meetings and contribute to the Plan of Care
• Provide emotional support, counseling, and guidance to patients and families
• Link patients and families to community resources, financial assistance and support services
• Document all patient interactions and assessments in the electronic medical record
• Support discharge planning and coverage for full-time social workers during absences or leave
