Advantage Hospice in Mechanicsburg, PA seeks a compassionate Hospice Social Worker to join our interdisciplinary team. You will provide emotional, spiritual, and psychosocial support to patients and families, advocate for their wishes in IDG meetings, and connect them with community resources.

Qualified candidates include MSW from CSWE-accredited programs with 1 year health care experience, or BSW with supervision and 1 year health care exposure; strong communication and crisis counseling skills

Qualificações

• MSW from a CSWE-accredited school with at least one year of health care experience.
• BSW from a CSWE-accredited program with at least one year of health care experience (supervised by an MSW).
• Bachelor's in psychology, sociology, or related field plus one year of health care social work experience (MSW supervision).

Responsabilidades

• Deliver psychosocial, spiritual, and emotional support to hospice patients and families.
• Perform psychosocial, spiritual, and emotional assessments, and develop care plans.
• Provide individual and family counseling, including crisis intervention.
• Participate in bereavement support and IDG meetings as an advocate.
• Maintain timely documentation and develop community outreach relationships.
