Advanced Hospice LLC is seeking a Full-Time Medical Social Worker (MSW/LCSW) to provide psychosocial support, resource coordination, counseling, and guidance to patients and families as they navigate life-limiting illness.

The MSW will work both independently and as part of an interdisciplinary team, developing patient-centered goals and coordinating community resources to improve quality of life for patients and their families.

Qualifications

• MSW or LCSW required.
• Strong psychosocial assessment skills.
• Ability to develop meaningful interventions and patient-centered goals.
• Experience in hospice or palliative care preferred.
