Job summary

Pruitthealth Corporation is seeking a Hospice Social Worker (MSW) to deliver psychosocial support to patients and families within the hospice setting. You will collaborate with the interdisciplinary team to develop and update care plans, provide crisis intervention, and assist with financial/insurance guidance.

Responsibilities include completing psychosocial assessments, counseling families through grief and loss, coordinating community resources, and documenting all services in the patient

Qualifications

• Master’s degree in Social Work (MSW) required.
• Current license and good standing with state licensure.
• One year experience in hospice social work preferred.

Responsibilities

• Provide psychosocial assessments and counseling to patients and families.
• Assist with financial and insurance benefits explanations and resources.
• Participate in care planning with the interdisciplinary team.
• Conduct crisis intervention and suicide risk assessment.
• Provide bereavement services and referrals as needed.
