VitalCaring is seeking a Master’s Social Worker (MSW) in Hospice to provide emotional, psychosocial, and practical support to patients and families navigating terminal illness and end-of-life care.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Meets applicable hospice personnel qualifications.
• 1+ year of MSW hospice experience preferred.
• Valid driver’s license and reliable transportation for home visits.

Responsibilities

• Assess the psychosocial needs of patients and families facing terminal illness.
• Provide counseling, emotional support, and guidance throughout the hospice journey.
• Identify psychosocial risks and provide interventions and crisis support when needed.
• Help patients and families navigate emotional and practical end-of-life challenges.
• Provide culturally sensitive support reflecting individual beliefs, traditions, and family dynamics.
