Job summary

Transitions Hospice is seeking a Licensed Social Worker to provide psychosocial support for patients and families across the hospice journey. You will conduct assessments, offer ongoing counseling, and connect families with community resources while ensuring compliance and meticulous documentation.

You will collaborate with an interdisciplinary team and participate in on-call coverage, with a focus on compassionate, patient-centered care in the Marshall, Mattoon area and surrounding regions.

Qualifications

• Master's degree in Social Work; licensure as required by state.
• Minimum of 2+ years social service experience in a health care setting or social services agency.
• Experience in hospice, palliative care, or related healthcare setting preferred.
• Valid driver's license and willingness to travel locally for patient visits.

Responsibilities

• Conduct initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers.
• Provide ongoing emotional support and counseling throughout the care journey.
• Identify and connect families with community resources and financial assistance.
• Guide families through Advanced Directives with clarity and sensitivity.
• Assist with facility placement for patients when appropriate.
• Maintain accurate clinical documentation at the patient’s residence and in agency records.
• Collaborate with interdisciplinary team meetings to shape holistic care plans.
• Participate in on-call duties on evenings and weekends as needed.
