Shalom Hospice seeks a compassionate Hospice Social Worker and Bereavement Coordinator to join our interdisciplinary team in Knoxville. This dual role provides emotional and psychological support to patients and families, while guiding bereavement processes and education.

The position involves counseling, group sessions, care-plan collaboration, and travel within the service area. Strong MSW credentials (LCSW preferred) and confidentiality are essential.

Qualifications

• Master's degree in Social Work (MSW) from an accredited program.
• Licensure as an LCSW in Tennessee.
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.
• Ability to travel within a designated service area.

Responsibilities

• Assess emotional and psychological needs of patients and families; develop bereavement plans.
• Provide counseling, including individual and group sessions.
• Facilitate support groups and educational programs for patients and families.
• Collaborate with the interdisciplinary team to develop care plans.
• Educate patients and families on end-of-life options and advance care planning.
• Maintain accurate documentation of patient interactions and services.
