Job summary

Qualifications

• Graduate of an accredited Nursing program.
• Active RN license as required by state regulations.
• Current BLS certification required.
• Previous hospice, palliative care, home health, or oncology experience preferred.
• Strong communication, assessment, and compassionate patient-care skills.

Responsibilities

• Assess patients and develop individualized hospice care plans.
• Manage pain, symptoms, medications, and comfort needs.
• Coordinate care with physicians, caregivers, and the hospice interdisciplinary team.
• Educate patients and families on disease progression and end-of-life care.
• Monitor patient status and update care plans as needed.
• Maintain accurate clinical documentation and hospice records.
