Qualifications

• Current, unrestricted RN license in the state of practice.
• Minimum of one year of nursing experience.
• Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area.

Responsibilities

• Complete comprehensive nursing assessments evaluating physical, emotional, and psychosocial needs.
• Develop and manage individualized care plans with patients, families, and interdisciplinary team.
• Monitor patient progress and adjust plans to maximize comfort and quality of life.
• Serve as the primary clinical resource for patients and families during hospice care.
• Administer medications safely and monitor effectiveness and side effects.
• Provide education to patients and caregivers on medications, symptom management, and disease progression.
• Identify changes in condition early and implement appropriate interventions.
• Build trusting relationships through compassionate communication and active listening.
• Provide emotional support and guidance during discussions on illness, grief, and end-of-life planning.
• Help navigate goals of care while honoring values and preferences.
