Responsibilities

• Provide high-quality bedside care to patients and their families.
• Deliver compassionate care in patients' homes, assisted living communities, and skilled nursing facilities.
• Manage patient symptoms with clinical expertise and empathy.
• Support patients and families throughout their hospice journey.
• Educate patients and families on disease progression, end-of-life care, self-care techniques, and nutrition.
• Collaborate with the interdisciplinary team to ensure comprehensive care.
• Document assessments, progress notes, care plans, and physician orders.
