Job summary

HopeHealth, Inc. is seeking a Hospice Registered Nurse (RN) Float for a part-time 24-hour day shift in Providence, RI. The RN will support the Massachusetts hospice team and handle a caseload assigned by the Patient Care Manager.

The role includes assessments, care planning, patient/family education, and coordination with the interdisciplinary team to ensure continuity of care. Part-time benefits apply; BSN preferred with 1–3 years’ experience.

Qualifications

• RN required; BSN preferred.
• 1-3 years of related experience.
• Strong nursing assessment, clinical skills and communication.
• Home care and/or related experience preferred.

Responsibilities

• Conduct initial and ongoing assessments of patient needs and plan of care.
• Implement and revise the individualized plan of care and discharge plans as appropriate.
• Educate patients and families about disease process, self-care techniques, medications, and diet as ordered.
• Initiate preventive and rehabilitative nursing procedures.
• Coordinate care with interdisciplinary team and family to address needs.
