Job summary

Qualifications

• Active, unrestricted RN license in California and current BLS certification.
• Clinical experience in hospice, palliative care, home health, or related settings with pain/symptom management.
• Ability to perform comprehensive patient assessments, develop and implement care plans, and provide nursing interventions.
• Strong communication and interpersonal skills to educate patients and families and collaborate with care teams.
• Proficiency in electronic health records (EHR) documentation and adherence to hospice regulations.
• Ability to work on-site in Burlingame, CA and manage visits and schedules effectively.
• Comfort with end-of-life care and providing culturally sensitive, non-judgmental support.
• Preferred: CHPN certification and familiarity with Medicare hospice guidelines.

Responsibilities

• Provide direct hospice nursing care to patients, including comprehensive assessments and symptom management.
• Manage pain and symptoms, administer medications, and coordinate individualized care plans with the interdisciplinary team.
